The task of the plastic surgeon in the treatment of obese patients is essentially two types of approaches: the first, what is usually adopted, is to surgically treat the unsightly remnants at the end of the weight loss program, the former obese patient who finished with a suitable weight loss diet therapy, medication or bariatric surgery, almost always presents significant flaws in the various parts of the body, whose gravity is proportional to the gravity of ' obesity starting to lose weight and other factors such as age and the elasticity of the skin. In fact, frequently, particularly for major weight loss achieved in the medium-short time, the skin does not pull together as the fat disappears, and then folds can form large unsightly and disabling.
It is surgery in which the masses are removed excess skin, fat especially in the abdomen, inner thighs, arms and breasts.
In some cases, the obese undergoing weight loss therapy has, in addition to excess skin, still areas of localized fat that does not suffer from the effect of dietary treatment and therefore also require surgery to be definitively eliminated, for example with a single intake of fat tissue.
The purpose of these interventions is therefore primarily to obtain psychological and aesthetic benefits important to encourage the patient to maintain an adequate lifestyle with healthy eating habits, but many cases, however, also give advantages in terms of functionality.
The other type of approach to the obese patient, by the plastic surgeon, is the (increasingly rare) pre or intra weight loss, ideal for those selected cases that require the prior removal of the adipose masses so large as to hinder or even prevent the movement and these masses of fat, usually the abdomen, can be so debilitating for the patient, the the point of rendering ineffective any weight loss program undertaken.
The latter type of patient requires, in terms of anesthesia, special attention, especially in the postoperative period, it often requires a short stay in intensive care, up to normalization of vital functions, it is often because of people with body mass index still very high and therefore frequently carriers of all issues related obesity: cardiac disorders, hypertension, respiratory and metabolic (diabetes) and vascular disease.
